Introduction

Salmonella Typhi, a bacterium that only infects humans and is spread through the fecal-oral route, causes Typhoid fever. In low- and middle-income nations, typhoid fever is a serious public health issue. In Southeast Asia, typhoid fever affects children and young people more frequently. Globally, 14.3 million instances of enteric fever were reported in 2017, with 76.3 percent of those cases being typhoid fever, which has a 0.95 percent case fatality rate (CFR).

Early detection and prompt administration of an efficient antibiotic can be crucial steps in lowering enteric fever-related illness and mortality. The CFR varies from 10% to 20% in cases that are left untreated or treated with the wrong antibiotics, compared to just 1% to 4% in patients that receive good antibiotic treatment according to sagepub website. Background

Poor hygiene has been linked to enteric fever, which is an endemic disease in South Asian nations and is caused by Salmonella enterica. This study aims to detect the status of multi- and very drug-resistant Salmonella isolates and examine the present antimicrobial resistance trend in Nepalese Salmonella isolates.

Methods

We collected blood cultures from people at study hospitals who had enteric fever suspicions. Antibiotic susceptibility tests were performed on the Salmonella isolates, and the trend in antimicrobial resistance was assessed.

Conclusions

Cephalosporins and other first-line antibiotics were most effective against the Salmonella isolates. Salmonella resistant to fluoroquinolones was abundant, although their overall trend seemed to be reducing. Additionally, beginning 2017, the overall number of S. Paratyphi cases has decreased. There were no isolates of S. Typhi that were exceedingly drug resistant, and only a small number of isolates were multidrug resistant.
